ADDITIONAL INFORMATION: 

The facility was licensed in the commonwealth of PA, Dept. of welfare as a personal care home (Title 55, PA Code: Chapter 2620-personal care home services, cert. Number 116540 dated Feb 14 2003 valid through Feb 14 2004) with capacity for 88 occupants.  However, from a functional standpoint 70 residence is deemed maximum capacity.  The former license was deemed to be conditional due to required building repairs.  Reportedly the license was revoked in April 2007. 

The structure is a 21,232 sq. ft., 4 level bank grade masonry & steel personal care facility that was reportedly constructed in 1959.  The exterior building is a combination of painted concrete block, painted stucco and stone veneer.  The floors appear to be steel reinforced poured concrete.  The buildings roof structure appears to be a built up insulated rubber membrane with a parapet gutter system for storm water control. 

The interior of the building reflects an institutional style with a combination of painted and papered drywall, suspended drop tile ceilings and a combination of vinyl tile and carpeted floor coverings.  Incandescent and fluorescent light fixtures were noted throughout.  Electric is provided by a 44 Amp, 3 phase service that appears adequate for the facility.  Heat is provided by a central oil fired furnace, 2 zones, with electric baseboard back-up units in some areas of the building.  There is no central air-conditioning.  Domestic hot water is supplied by an oil fired commercial water heater.  The building is also fully sprinkled with a wet system.

The buildings floor plan consists of a main entrance, kitchen, dining area, activities room, offices and other ancillary areas on the first floor.  The ancillary areas include: housekeeping, public lavatories, laundry room, and an area housing mechanical systems.  The second level contains 16 rooms, a sitting room, 7 half baths, 2 full baths, a public restroom, and a pharmacy.  The third floor contains 15 rooms, a sitting room, 3 half baths, 1 full bath, and 1 public restroom.  The fourth floor contains 22 shared guest rooms, 12 full baths.  There is 1 elevator and 2 stairwells that provide access to each floor.

Site improvements consist of 3 private paved asphalt streets with 2 cul-de-sacs, asphalt paved vehicle parking areas, under ground water, sewer, and electric lines, and storm water management facilities.  The overall condition and functional utility of the site improvements appears AVERAGE.
